路 Positive status for Human Immunodeficiency Virus (HIV) or the treatment of this condition
路 Acquired immune deficiency syndrome (AIDS) or the treatment of this condition
路 Hepatitis C or the treatment of this condition
A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following:
路 Cachexia or Wasting Syndrome
路 Severe, debilitating, Chronic Pain
路 Severe nausea
路 Seizures, including but not limited to those characteristic of epilepsy
路 Severe and persistent muscle spasms, including but not limited to those characteristic of Multiple Sclerosis or Crohn's disease
路 Agitation related to Alzheimer's Disease
路 PTSD (18 years old and older)
Any other conditions approved in the future.

Specifically these patients are looking to apply to the medical marijuana program (in RI) for the first time or to renew their license with a providers certification. We desire providers who are willing to provide certification as appropriate and committed to providing time efficient visits without diminishing quality. We need team oriented applicants who are trustworthy and committed to the position.
