POSITION OVERVIEW
The Licensed Practical Nurse (LPN) position will support a physician and family nurse practitioner in the day-to-day clinical operations in the primary care setting. Collaboration between the LPN and practitioner will see success in encountering patients for primary care, addiction medicine, mental health, medical cannabis certification and weight loss management.
RESPONSIBILITIES
Collaborates with practitioners, management, and medical assistants in a teamwork environment to fulfill clinical responsibilities
Collaborates with practitioners and management in meeting clinical performance objectives
Reviews, responds, and distributes clinical correspondence via fax, mail, or electronic messages as needed
Oversees and maintains patient flow for physicians/practitioners
Performs general inventory as assigned per management
Ensures that all laboratory specimens are collected, processed, and documented
Observes, measures, records, and reports data relating to a patient’s health status
Provides administrative support including scheduling patient appointments, checking out patients, and generating referrals as ordered by practitioner
Vitals patients and administers vaccinations, therapeutic injections, and other medical procedures as ordered by the practitioners
Assists management in addressing quality initiatives including coordinating, reporting and completing goals set by the Quality Management Department
Manages prescription refill requests and follows prescription refill protocol
Conducts approved prior authorizations for medications, test, and/or procedures
Provides patient education as instructed by practitioner
QUALIFICATIONS AND EDUCATIONAL REQUIREMENTS
Active New York State LPN License (Required)
One Year of Medical Experience (Required)
Bilingual in English & Spanish at a conversational level is required
Associate's Degree in Nursing (Preferred)
BLS Certification (Preferred)
