Work Schedule NE Portland Location - Shift starts at 4:45 AM to 12:30 PM on scheduled work weekdays, 3 to 4 days per week and alternating Saturdays at 6:45 AM to 10:00 AM.
Responsibilities and Duties Perform daily assessments of patients' level of consciousness (check for S/S withdrawal) to dispense medication, monitor and document per DEA guidlelines in EHR, their daily medication. Collect and document medical history on all new patients and annually on all existing patients. Administer TB testing, random breathalyzers, observe and collect Urine Drug Screens. Coordinate Care with outside agencies for patients with both medical and mental health concerns; i.e. PCP prescribing Rx's, MH providers, DHS, PO's and other community resources.
Qualifications and Skills RN or LPN with current Oregon License and a desire to work in the field of addiction. Good verbal and written communication skills. Responsible, respectful, and professional towards patients. Working knowledge of Word and Excel.
Salary Wages $34 per hour or higher depending on experience.
All Locations closed Sundays and major holidays. Part-time (24-27hrs per week) with potential for full time (32-36 hours per week). Offers of employment will be contingent upon drug screen results and background check. Work Remotely - No. Onsite work only.

· Under the direction/supervision of the Medical Director, will administer and/or oversee the administration of methadone, in accordance with OAR’S, Federal Regulations, CARF, DEA guidelines,Will document and/or oversee documentation of all administered dosages, by the best means available, whether that method is electronically, or manually.· Documents patient care services by utilization of best charting practices (with either paper or electronic records, whichever may be in use at the treatment facility at the time), and the ensuring the integrity of all other medical charting that may occur.Recognizes office medical emergencies and coordinates treatment within the multidisciplinary team; and follows licensed medical professional guidelines.Oversee accuracy of Nurses daily audits of the medications that are supplied to patients and maintain medication in secure storage. Maintain and have overall responsibility for monthly audits, completing inventory check, and investigate discrepancies with methadone.
-Follows established protocols and documents all processes and procedures correctly.
-Reviews patient absent report, followup on all medical paperwork,patient dose changes, lab results, physician orders, phase/level changes, and verbal orders. Coordination with hospitals, Courtesy dose locations, Jails, and PCP's to assure adherence to IHC protocol
-Places and/or oversees the placing of orders for methadone and other medication supplies.This position will ensure that there is sufficient scheduling coverage for nursing staff, they are effectively cross-trained for such ordering; receipt of ordered medications is facilitated with accuracy and efficiency upon the arrival of the medication.
- Promotes the delivery of quality care to patients by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; and complying with applicable rules, including state licensing laws(OARS), federal rules governing substance abuse treatment centers and applicable IHC policies and directives.
-Ensures that the Dispensary is opened and closed in accordance with policy; staffing protocols are followed and establishes the nursing schedule; reviewing of all take-home medication changes and documents, evaluates Diversion Control procedures (patient callbacks, etc.)
-Conducts and/or oversees the administration of tests ordered by the physician including appropriate referrals for lab tests(blood) as required; patient vital signs; TB tests when required; and urine laboratory testing-Schedules and screens patients to be seen by the Medical Director and will assist the Medical Director with appropriate History and Physical assistance, as needed and required.-Actively participates in Treatment Team meetings and staff meetings; acts always in the best interest of the patients, the program and IHC.-Reviews medical records of potential patients for appropriateness to the MAT program.
-Identifies issues or concerns regarding client safety and well-being.
-Monitors and tracks, with Medical Director approval, the Prescription Monitoring Program based on the IHC policy and state mandatesProvides, HIV, Pregnancy, TB, and as needed medical information to patients and staff-Maintains doctor meeting notes concerning continuity of care for complicated cases.-Coordinates care as needed with outside physicians to ensure proper protocols are followed for patients. Maintains dialogue with other health care professionals (dental, mental health, chemical dependency, etc.) to promote and facilitate comprehensive healthcare services i.e. whole person-centered care throughout the recovery environment.-immediately reports any concerns to the Program Director-Provides training and continuing education to nurses-Tracks and monitors outside prescriptions by performing weekly/ monthly medication counts with patients, and tracking patient compliance with RX registration.Available on-call for consultation of patient evaluations-Performs other duties as required.
